For me it's got to be Chekov's gun: If you say in the first chapter that there is a rifle hanging on the wall, in the second or third chapter it absolutely must go off. If it's not going to be fired, it shouldn't be hanging there. I guess it's more of a general story-telling thing, but most of the media I consume is movies these days so that's why it bothers me. I understand that it's just a matter of clean and efficient scripting, but I really wish I'd never heard of it.
