LODI — Dr. Kyle Robert Krueger, a Lodi physician on probation since 2009 for a string of arrests involving driving under the influence of alcohol, has had his license to practice medicine permanently revoked by the Osteopathic Medical Board of California.The effective date of the revocation is May 22. In addition to losing his license, Krueger, 58, was ordered to pay $10,000 to cover the costs of the medical board’s investigation and prosecution of his case.
